Policies & Procedures during COVID-19
Please wear a mask. If you do not have one, one will be provided by the office. Your mask should be on at all times, except for if your treatment or consultation requires it to be removed.
Complete paperwork online. 2-3 days before your appointment, you will receive a text message asking you to complete a Patient demographic request and a COVID-19 prescreen questionnaire. Please have this completed PRIOR to your appointment. If you are having technical issues please contact our office ASAP.
Hand Sanitizer. When you arrive, please use the hand sanitizer provided.
Please attend your appointment alone. If a spouse or translator must accompanying you to the appointment they must also wear a mask and asked to sanitize their hands.
If you or anyone in your household is experiencing any symptoms of COVID-19, please call our office to reschedule.
For your safety and the safety of others, Due to COVID-19 our SPA return policy has changed. We can NO longer except product returns as of June 1, 2020.
